IMPORTER:
      Has resided abroad for at least one (1) year

MOTOR VEHICLE:
a. Left Hand Drive
b. Not exceed 3,000 kgs, GVW
c. Registered under the name of the qualified importer for at least six (6) months prior to the submission of the application to BIS

FOR PHILIPPINE PASSPORT HOLDERS
a. Completely Filled-up BIS Application Form
b. Notarized Affidavit of Undertaking
c. 2x2 Picture with Signature
d. Original or *Authenticated Copy of Complete Pages of Old and New Passport
e. Original or *Authenticated Copy of Car Title or Registration
f. Processing Fee of One Thousand Pesos (P1,000.00) for Cars and Six Hundred Pesos

FOR FOREIGN PASSPORT HOLDERS (13A AND 13G VISA HOLDERS ONLY)
a. Completely Filled-up BIS Application Form
b. Notarized Affidavit of Undertking
c. 2x2 Picture with Signature
d. Original or Authenticated Copy of Complete Pages of Old and New Passport with Valid 13a or 13g Visa as the Case Maybe
e. Original or Authenticated Copy of Car Title or Registration
f. Processing Fee of One Thousand Pesos (P1,000.00) for Cars and Six Hundred Pesos (P600.00) for Motorcycle
g. Alien Certificate of Registration (ACR) and Immigrant Certificate of Residence (ICR) (To be compiled and presented upon arrival of applicant in the Philippines)

FOR FOREIGN EXPATRIATES OF E.O. 226 "OTHERWISE KNOWN AS THE OMNIBUS INVESTMENT CODE OF THE PHILIPPINES"

b. Notarized Affidavit of Undertking

d. Endorsement or Certificate of Eligibility from the Board of Investments (BOI)
e. Processing Fee of One Thousand Pesos (P1,000.00) for Cars and Six Hundred Pesos (P600.00) for Motorcycle

* Authentication is required only when original documents cannot be presented.
*Non-original documents shall be authenticated by Philippine Consulate/Embassy abroad.

NOTE:
             After complying with all of he above requirements, please submit to the Bureau of Import Services, 3rd Floor, OPPEN Bldg., 349 Sen. Gil J. Puyat Ave., Makati City. Your documents will be evaluated and you will be issued a clearance/license/authority to your motor vehicle.
